Whatever you need, it starts here.
Every request you can make as a tenant, in one place, with the timeframe the law puts on it and a dated copy of the request to keep. You do not need to know which form you want. Tell us the situation and we will find it.
What do you need to do?
Pick the area, then the situation. Some of these start a clock the moment we receive them, so it is worth knowing which before you send anything.
The timeframes that actually apply
Four of these requests put a legal deadline on the landlord, and if the deadline passes without an answer the request is approved by default. Most tenants have never been told this, and most agencies do not volunteer it.
What the landlord has to answer by
Counted from the day after they receive the request, and if it is posted, add delivery time on top. Miss the deadline and the request is approved, or consent is deemed given, without them.
How much notice you have to give
Which one applies depends on the kind of tenancy you are in and why you are leaving.
Residential Tenancies Act 1987 (WA): pet requests s 50B, minor modifications s 50O, affixing furniture s 50J, major modifications s 50Y, notice from tenant ss 68, 69, 70A and 71AD. Checked against Consumer Protection WA in September 2026. The automatic approvals are real but they are not automatic paperwork, so keep your dated copy either way.
Everything you can do here
Sixteen forms plus the maintenance portal, grouped by what they are about. Anything marked with a document produces a completed, dated form you can keep.
Every form is set out in full below this list. Scroll down to the one you need.

Feedback and complaints
Nothing matches that. Try a plainer word, or call us on 0422 238 434 and we will sort it out with you.
Lease renewal
Ask to stay on. Send it about two months before your lease ends, which gives us time to put it to the owner and get an answer back before anyone has to make other plans. The questions below the basics are what we take to the owner on your behalf.
Notice to vacate
Formal notice that you are ending the tenancy. This one has a real legal shape to it, so the page works out how much notice you are giving and tells you if it is short before you send it. You get a completed, dated notice to keep.
Break lease request
Leaving before a fixed term ends. There is no prescribed form for this and no automatic right to do it, so it is a request rather than a notice. Costs usually apply. The earlier we know, the more of them we can usually avoid.
Change of occupant
Adding a partner, taking a housemate off the lease, or swapping one person for another. Anyone joining has to be assessed and added to the agreement, and anyone leaving stays liable until they are formally released.
Subletting request
Renting out a room, or the whole place while you are away. Written consent is required before anyone moves in, and consent cannot be unreasonably refused. Short-stay letting is a different thing again and usually needs council approval as well.
Pet request
Since the 2024 changes, a landlord cannot simply say no to a pet. There is a closed list of reasons for refusing, most of which need the Commissioner's approval, and the clock runs whether or not anyone looks at it.
Modification request
A minor modification is one of twenty-two specific things on a list in the Act, from a picture hook to painting a room, and it runs on a 14-day clock. Anything else is a major modification and runs on 28 days. Pick the wrong one and the clock does not start, so if you are not sure, say so and we will sort it.
Anchor furniture to a wall
Securing a bookcase, a chest of drawers or a television so it cannot tip onto a child or a person with a disability. This one has its own section of the Act and a very short list of reasons it can be refused: asbestos, heritage listing, or a strata by-law. Nothing else.
Inspection reschedule
The time we have set does not work. Give us two or three alternatives and we will move it. You do not have to be there for a routine inspection, but plenty of people prefer to be.
Trouble paying rent
Tell us before the payment is missed rather than after. Almost every option that exists, from a short payment plan to a rent variation the owner agrees to, depends on there being time left to use it. This goes to Daria directly.
Move-in check-in
A week or two in, once the boxes are mostly gone. The point of this one is the property condition report, which is the document that decides your bond at the other end of the tenancy, and the small faults that are easy to fix now and expensive to argue about later.
Document request
Copies of anything on your file. Most of it is sent the same day. A rent ledger for a bank or a rental reference usually takes a little longer because it is produced fresh.
Property grievance
A formal complaint, recorded as one. Dates and specifics are what make a grievance actionable, so put in more than feels necessary. If the matter is about safety, harassment or anything you would rather not have on the ordinary file, use the confidential form instead.
Sensitive or confidential matter
Safety, harassment, family violence, discrimination, or a complaint about us. This goes to Daria and nowhere else. You do not have to give your name, and nothing here is passed to the owner without your say-so.
Feedback on your property manager
Anonymous is fine. This is read, and it is the main thing that changes how we work.
General survey
Two minutes on how the tenancy is going. What comes back here is what we put in front of owners at renewal time, which is how most small improvements actually get approved.
You keep a copy of everything
Seven of the requests on this page produce a completed document the moment you send them: your answers, the wording the Act uses, the date, and a reference number. It is built in your browser, so you can save it or print it straight away.
These documents are our record of the request you made and the covering document we send to the owner. They are not the forms approved by the Commissioner for Consumer Protection under section 88C. Where a matter needs the approved form served on the owner, we serve it and send you the copy. You can download any of the approved forms yourself from Consumer Protection at any time.
